We are committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the goods and services provided through DVRMT. All policies are in accordance with the Massage Therapy Act, 1991, CMTO Standards of Practice, Personal Information Protection and Electronic Documents Act, 2000, and the Personal Health Information Protection Act, 2004.

Personal information is defined as information about individuals and including:

  • An individual's personal characteristics (e.g., gender, age, home address or telephone number, family status)
  • Health (e.g., health history, health conditions, health services received by them)
  • Activities and views (e.g., opinions expressed by an individual, an opinion or evaluation of an individual)

We maintain your privacy and confidentiality by using your information in these ways:

  • To develop a personlized treatment program for you
  • Get payment for your treatment (from OHIP, WSIB, private insurer or other if required)
  • Internal administration, planning, development
  • In compliance with legal and regulatory requirements
  • If required by law

Section 4

THIRD PARTY ACCESS

The College of Massage Therapists of Ontario may inspect records as a part of their regulatory activities in the public interest as mandated by law. Various government agencies (eg. Canada Customs and Revenue Agency, Information and Privacy Commissioner, Human Rights Commission, etc.) also have the authority to review documentation as a regulatory body. Your written consent will be required before submitting personal health information to non-regulatory bodies (eg. Insurance, Legal Council, etc.).

When you browse our website, we automatically receive your computer’s internet protocol (IP) address in order to provide us with information that helps us learn about your browser and operating system.

How do you get my consent?

When you provide us with personal information to complete an online form, we imply that you consent to our collecting it and using it for the specific required form only.

To protect your personal information, we take reasonable precautions and follow industry best practices to make sure it is not inappropriately lost, misused, accessed, disclosed, altered or destroyed.

If you provide us with your credit card information, the information is encrypted using secure socket layer technology (SSL) and stored with a AES-256 encryption. Although no method of transmission over the Internet or electronic storage is 100% secure, we follow all PCI-DSS requirements and implement additional generally accepted industry standards.
